When filling out the form, select the option that says you are "A member of a Postcard Group", then select "Temple Shaari Emeth (Manalapan)". Postcards will be sent as a group to Ellen Finkelstein after the deadline of July 1st (even though the form says you'll get them in 1-2 weeks) and then she will distribute them.
